Job Description
The Case Manager RN leads the coordination of a multidisciplinary team to deliver a holistic, person-centric care management program to a diverse health plan population. Acting as the single point of contact for members, caregivers, and providers, the RN Case Manager uses phone, email, text, and online messaging to support members across the care continuum. This role requires a balance of clinical expertise, care coordination, and compassionate communication to improve member outcomes and ensure continuity of care.
Key Responsibilities
- Lead and coordinate a multidisciplinary team (social workers, dietitians, pharmacists, clinical support staff, and medical directors) to meet member needs via telephonic/digital channels
- Assess member health, psychosocial needs, cultural preferences, and support systems.
- Develop individualized care plans, address barriers, and connect members with necessary community and clinical resources.
- Coordinate care transitions, discharge planning, and post-transition education.
- Deliver health education, self-management coaching, and support adherence to medication and care plans.
- Advocate for members, promoting self-advocacy and improved overall health outcomes.
- Document interactions accurately to support continuity and quality of care.
- Maintain professional standards and continuing education requirements (CCM)
Required Education & Experience
- Nursing diploma or associate degree in nursing (required); BSN strongly preferred.
- 3 years of clinical nursing experience (clinical, acute/post-acute, or community setting).
- 1 year of case management experience in managed care (preferred).
- Experience with telephonic and digital patient management (preferred).
Licenses & Certifications
- Current, active, and unrestricted Michigan RN license (required).
- CCM certification (required within 18 months if not already held).
- CCP certification (preferred).
