Job Description

Scope of work:
NYCCC-Queens Campus is seeking services of a valid licensed Speech-language Pathologist for children/adolescents. Services to be provided shall include, but may not be limited to, patient evaluations, treatment planning, speech therapy, consultation to staff, and related duties

• Documentation of evaluations and therapy must be completed per the Uniform Case Record and NYCCC policies.
• Service hours shall not exceed 15-20 hours per week with a maximum of 750 hours annually.
• Facility General hours are 8:30am to 5:00 pm, Monday – Friday.
- Scheduling is flexible and can be arranged as per the mutual needs of the facility and Speech Therapist.
• HIPAA/HITECH and other mandated training must be completed as determined by OMH/Facility.
• Speech Therapist must visibly wear facility issued badge/identification at all times during course of the work schedule.
• Speech Therapist is expected to integrate readily into the treatment team and interact with staff at all levels in their professional capacity.

Awarded candidates must complete the first four of the five mandatory forms required for clearance prior to interview.

The Health Clearance Form will be required of the selected candidate.

3) LDSS-3370 (SCR) Form
4) State Central Child Abuse Register Certification Status Form
5) Health Clearance Form
