Job Description:

We are seeking a highly skilled and experienced Director of Case Management to lead and oversee our case management team. The ideal candidate will possess a strong background in nursing and case management, with the ability to manage and coordinate patient care effectively.

Minimum Work Experience:

- Minimum 3 years RN case management experience (acute care) or Care Coordination experience required.
- Minimum 3 years management experience preferred.

Key Responsibilities:

- Lead and supervise the case management team to ensure high-quality patient care and compliance with healthcare regulations.
- Develop and implement case management policies and procedures.
- Collaborate with healthcare providers, patients, and families to facilitate comprehensive care plans.
- Monitor and evaluate patient outcomes to ensure optimal care and resource utilization.
- Provide education and support to case management staff.

Qualifications:

- Associate's Degree in Nursing required (BSN preferred).
- Current RN license to practice in NC (or multi-state compact license or by endorsement).
- CCM Certification completion within the first year of employment or transfer to the role.
- BLS Certification required.
- ACMA Certification preferred.
- Strong leadership and communication skills.
- Experience in case management and healthcare coordination.
