Job Description

Licensed Vocational Nurse is responsible for the delivery of patient care to assigned patients utilizing the nursing processes of assessment, planning, implementation, and evaluation. The licensed practical/vocational nurse is responsible for meeting the established standards of their profession as well as those established by the nursing department of the facility in which they are employed. He/she Is responsible for meeting the established standards of his/her profession and for practicing within the scope of licensure. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Demonstrates knowledge of age specific patient populations and cultural diversity during the provision of patient care. Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care. Promotes an environment of care.

DUTIES AND RESPONSIBILITIES
· Tasks can include taking vital signs, giving injections, performing routine tests.
· Maintaining medical charts, dressing wounds, helping with patient bathing and performing Some clerical work. · Considerable knowledge of the care and treatment of patients and special procedures that Apply to practical nursing.
· Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others; able to teach patients and families in accordance with the nursing plan of care
· Able to use sensory and cognitive functions to process and prioritize information, treatment, and follow-up
· Able to record activities and document interventions
· Able to use computer and learn new software programs · Collaborate with other health care providers and provides education to patients and/or significant others (while in compliance with HIPAA)
· Performs related duties as requested

Department: Primary Care

Additional Information: 4 month assignment with potential to extend; Approx 36-40hrs per week. Highly preferred experience – Ambulatory Care Experience, KP, & Health Connect. Assignment Details: Assignment – providing 1:1 back office support to physicians (rooming patients, vaccinations, etc.)

Schedule: 8:15a-5:15p Mon-Fri; with the possible Saturdays- Will be given a week day off and advanced notice
