Position Summary: We’re looking for a compassionate, dependable Licensed Practical Nurse to join a healthcare team in Brooklyn, New York. In this full-time, permanent role, you’ll provide hands-on nursing care, administer medications, monitor patient conditions, and partner with a multidisciplinary team to promote safety, wellness, and continuity of care.

Key Responsibilities:

• Administer prescribed medications, treatments, and immunizations
• Monitor patient conditions, take vital signs, and promptly report changes in health status
• Provide wound care and help manage chronic health conditions
• Assist patients with activities of daily living, including hygiene, mobility, and nutrition
• Accurately document assessments, treatments, and medications in the Electronic Health Record
• Educate patients on medications, wellness, treatment plans, and self-care
• Collaborate with nurses, providers, and other healthcare professionals to support individualized care plans
• Follow infection prevention procedures, maintain a safe care environment, and respond quickly to urgent situations

Qualifications:

• Active, unrestricted Licensed Practical Nurse license in any U.S. state
• Previous LPN experience preferred
• Experience in urgent care, behavioral health, correctional healthcare, or similar clinical settings is a plus
• Strong documentation skills and attention to detail
