Nurse Practitioner (NP)

Location: Portsmouth, NH
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Job Type: Full-Time
Pay: $80–85 per hour (DOE)

Overview:
Our medical practice in Portsmouth is seeking a Nurse Practitioner (NP) to serve as a key clinical decision-maker for our residents. You will lead medical care alongside a dedicated care team.

Key Responsibilities:

• Manage admissions and perform advanced clinical assessments for residents with complex, chronic, and acute conditions
• Manage chronic diseases such as heart failure, chronic obstructive pulmonary disease (COPD), and diabetes through ongoing monitoring and treatment adjustments
• Prescribe, adjust, and manage medications, including reviewing polypharmacy and monitoring for adverse effects
• Provide wound care and pressure injury management
• Develop and update individualized care plans in collaboration with nursing, therapy, and social work
• Coordinate discharges and continuity of care, and participate in interdisciplinary team meetings
• Educate residents and families on diagnoses, treatment options, and self-management
• Document assessments, orders, and progress notes accurately in the electronic medical record (EMR)

Qualifications:

• Active, current Nurse Practitioner (NP) licensure in New Hampshire
• Skilled nursing facility, long-term care, or geriatrics experience
• Experience with admissions, medication management, chronic disease management, and discharges
• Comfort providing care to adult and geriatric populations
• Experience with electronic medical records preferred
