Nurse Practitioner (NP)

Location: Lexington, KY
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Job Type: Permanent, Full-Time
Pay: $70–75 per hour (DOE)

Overview:
Our medical practice in Lexington is seeking a Nurse Practitioner (NP) to serve as a key clinical resource for our residents. You will work closely with a dedicated care team to deliver coordinated, high-quality care.

Key Responsibilities:

• Manage admissions and perform thorough clinical assessments for residents with complex medical needs
• Manage chronic conditions such as congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), and diabetes, adjusting treatment plans to maintain stability
• Prescribe, adjust, and reconcile medications, ensuring safety, efficacy, and adherence to current guidelines
• Provide acute and post-acute care management, including oversight of transitions and discharges
• Document encounters, orders, and care plans accurately in the electronic health record (EHR)
• Lead and participate in interdisciplinary care conferences with physicians, nurses, therapists, and social workers
• Educate residents and families on diagnoses, treatment plans, medications, and self-management
• Contribute to quality improvement initiatives and support regulatory and compliance standards

Qualifications:

• Active, current Nurse Practitioner (NP) licensure in Kentucky
• Minimum 1 year of skilled nursing facility or long-term care experience
• Experience with admissions, medication management, chronic disease management, and discharges
• Comfort providing care to adult and geriatric populations
• Experience with electronic medical records preferred
