Licensed Vocational Nurse (LVN)
Thousand Oaks, CA

Overview:
Our specialized adolescent behavioral health and treatment program in Thousand Oaks is seeking a compassionate Licensed Vocational Nurse (LVN) to support and mentor adolescent patients through a critical stage of their care. This part-time role offers a consistent Monday–Friday morning schedule and the chance to make a lasting impact within a close-knit clinical team.

Position Offers:

• Schedule: Monday–Friday, 9:00 AM–1:00 PM (20 hours/week)

Key Responsibilities:

• Conduct patient clinical assessments and routine physical health checks
• Maintain medication administration records and update the electronic medical record (EMR)
• Prepare accurate SOAP notes and carry out physician orders
• Collaborate with medical providers to support individualized patient care plans
• Coordinate clinical appointments and assist with patient transportation logistics

Qualifications:

• Active, current Licensed Vocational Nurse (LVN) licensure in California
• 1 year of professional LVN experience preferred
• Previous experience in pediatrics, adolescent care, behavioral health, or mental health settings preferred
• Strong clinical documentation, communication, and interpersonal skills
