Licensed Practical Nurse (LPN)
Near Concord, NH

Overview:
Our facility near Concord is seeking a Licensed Practical Nurse (LPN) to play a vital role supporting clients through withdrawal management, one of the most critical stages of their recovery journey. New graduates and experienced LPNs alike are encouraged to apply and grow their clinical skills in a specialized setting.

Position Offers:

• Schedule: Monday, Tuesday, Friday, and Saturday
• Competitive pay: $79,040–95,680 per year (DOE)

Key Responsibilities:

• Perform intake assessments, including gathering health histories and evaluating current symptoms and conditions
• Conduct COWS (Clinical Opiate Withdrawal Scale) and CIWA (Clinical Institute Withdrawal Assessment) withdrawal assessments
• Provide medical monitoring for clients undergoing withdrawal, including vital signs and promptly reporting changes in status
• Administer medications safely and accurately, following facility protocols and provider orders
• Support development of individualized care plans in collaboration with RNs and other healthcare professionals
• Document all patient interactions and interventions accurately in the EHR
• Collaborate with the interdisciplinary healthcare team, including participation in care conferences

Qualifications:

• Active, current Licensed Practical Nurse (LPN) licensure in New Hampshire
• Comfortable performing withdrawal assessments and medical monitoring
• Strong communication and patient education skills
