Registered Nurse (RN) – Hospice Case Manager
Irving, TX

Overview:
Our established hospice organization in Irving is seeking an experienced hospice Registered Nurse (RN) Case Manager to provide compassionate end-of-life care to patients in their homes and in facilities. This full-time role offers a consistent Monday–Friday schedule with minimal admissions responsibilities.

Position Offers:

• Schedule: Monday–Friday
• Competitive salary: $90,000–95,000 per year (DOE)

Key Responsibilities:

• Provide hospice case management for patients in homes and facilities across the coverage area
• Perform comprehensive full-body assessments during patient visits
• Manage medication refills and coordinate with providers as needed
• Oversee and support Certified Nursing Assistants (CNAs) during patient visits
• Participate in Interdisciplinary Group (IDG) meetings to plan and update patient care
• Support a very minimal admissions rotation, with Clinical Liaisons handling most admissions and General Inpatient (GIP) cases
• Maintain accurate, timely clinical documentation

Qualifications:

• Active, current Registered Nurse (RN) licensure in Texas
• Minimum 1 year of recent hospice RN case management experience
• Strong clinical assessment skills and independent judgment
• Valid driver’s license and reliable transportation required
