Job Description – Case Manager

Case Manager will have a caseload of around 5 families. Case Manager will ensure safety and stabilization of the families assigned to them. The position is 35- 40 hours per week, full-time, with evenings and weekends required as needed. The overarching goal of Case Manager is to avoid out-of-home placement of youth on their caseload.

Duties – Case Manager

• Case Manager will make home visits to family on caseload and assess safety
• Case Manager will provide on-call services to family
• Case Manager will conduct crisis intervention with family as needed
• Case Manager will assist family in creating a plan to improve family functioning
• Case Manager will provide parenting skills counseling, stress management, anger management, and communication skills development as needed
• Case Manager will connect the family with community resources

Requirements – Case Manager

Bachelor's Degree in a related field with valid driver’s license and a reliable vehicle

AND

At least 1-2 years of experience working with children and/or adults with behavioral health/mental health disorders

Job Type: Full-time

Salary: $18.00 - $21.00 per hour

Schedule:

• 8 hour shift
• Weekend availability

• Bachelor's in Human Services(Required)
• At least 12 behavioral Health college credits (Required)

License/Certification:

• Driver's License AND a reliable vehicle (Required)
