Summary
Provide clinical support to program participants in addiction and mental health services within residential or outpatient settings. Deliver therapeutic counseling through insight and targeted interventions, maintaining a caseload and offering both group and individual counseling as specified by program standards. Ensure the program aligns with participants’ individual goals and follows the established recovery model. Demonstrate respect and dignity toward clients and staff, consistently exhibiting a caring and supportive attitude while positively representing the program. Safeguard participant information and uphold program values as outlined in the employee handbook.
Essential Duties and Functions
Therapy
•Collaborate with participants to set and achieve recovery goals related to medical, psychiatric, and substance use issues.
•Conduct individual and group counseling sessions.
•Support aftercare planning and follow-up for participants.
Caseload Management
•Assess participants’ progress and report to funders/referral sources as requested during authorization and re-authorization reviews.
•Engage as a member of the support team in clinical case conferences.
•Manage active caseload, ensuring ongoing treatment engagement.
•Track treatment plans and required documentation in line with funder and program guidelines.
•Contribute to clinical group meetings focused on treatment goals, progress, and discharge planning.
Administrative
•Document in electronic medical records per agency standards.
•Maintain an up-to-date scheduling calendar.
•Perform other duties as assigned.
Qualifications
•Bachelor’s degree in Social Work, Psychology, or a related field.
•Certification may be required within a specified timeframe based on program license needs.
•Minimum of two years’ experience working with individuals with co-occurring disorders.
•Proficient in business software and electronic health record systems.
