Care Coordinator Manager

First Place Pediatrics | Multiple Locations | Full-Time | Exempt Pay: $75,000–$85,000/year, plus bonuses

About the Role

First Place Pediatrics is looking for an experienced Care Coordinator Manager to lead our care coordination team and help us deliver exceptional, compassionate care to the clients and families we serve. In this role, you'll manage a team of Care Coordinators along with the RNs and LPNs who support them , making sure every client receives seamless, high-quality care while your team gets the coaching, structure, and support they need to succeed.

This is a hands-on leadership position for someone who thrives on building strong teams, solving problems before they escalate, and being the steady point of contact clients and families can count on.

What You'll Do

• Lead and develop your team. Coach, train, and manage performance for a high-performing care coordination team. Oversee recruitment, onboarding, workload distribution, and resource allocation, and foster a positive, collaborative work environment.
• Own client relationships. Serve as the primary point of contact for client and family inquiries and concerns, resolving issues quickly to maintain high satisfaction.
• Manage clinical staff. Provide ongoing coaching and support to RNs and LVNs to ensure care delivery consistently meets client needs and quality standards.
• Coordinate care planning. Partner with nursing staff, caregivers, and families to implement and adjust individualized care plans that align with client goals and regulatory requirements.
• Ensure quality and compliance. Regularly assess service delivery against care plans, safety protocols, and regulations; run quality assurance check-ins; oversee accurate documentation and support audit/inspection readiness.
• Manage staffing and scheduling. Work with Care Coordinators to keep staffing levels aligned with client needs and resolve scheduling issues as they arise.
• Support billing and insurance. Help ensure timely, accurate claims submission and work with insurance providers to verify coverage and resolve billing discrepancies.
• Drive operational excellence. Support intake, assessments, and care plan development; assist with training new team members; and coordinate with Area Directors and Business Development on Start of Service for new patients.
• Champion the client experience. Collect and act on client and family feedback to continuously improve service quality, and keep leadership informed on client status, staffing, and operational concerns.
• Support talent acquisition. Build and leverage local relationships in the community to source and attract qualified candidates, helping drive hiring for the care team.

What You Bring

• Bachelor's degree (preferred)
• 1-3 years in a leadership role
• Strong knowledge of healthcare industry trends, regulations, and key stakeholders (hospitals, medical practices, insurance providers)
• A proven track record of driving growth and results in a medical or healthcare setting
• Excellent negotiation, presentation, and communication skills
• Strong leadership abilities with experience managing cross-functional teams
• Strategic mindset with the ability to spot and act on opportunities
• Proficiency in Microsoft Office Suite and other business tools
• Willingness to travel within the assigned region as needed
