Job Description
Our client is seeking a dedicated Physician (MD or DO) to provide exceptional care to residents in senior living facilities across Bucks County, PA. You will manage chronic conditions, lead health assessments, and collaborate with a skilled clinical team to ensure every patient receives personalized, high-quality care. This is a hands-on, meaningful role where you can make a real difference in the lives of older adults while contributing to the growth and innovation of the practice.
Responsibilities:
- Provide compassionate primary care services for a geriatric and/or disabled patients including comprehensive and focused physical examinations, health assessments, diagnoses, and treatment of acute and chronic illnesses
- Manage chronic conditions such as diabetes, hypertension, congestive heart failure, arthritis, chronic obstructive pulmonary disease, and thyroid disorders
- Assess need for appropriate lab tests, order and interpret results
- Collaborate with medical specialties through appropriate referrals
- Assist in coordination of community resources to meet needs of individuals and families, as well as appropriate follow-up.
- Educate patients on health promotion throughout the aging process
- Establishes and monitors appropriate levels of care for center patients.
- Completes all medical record documentation prior to the end of shift.
- Works with the Medical Director to ensure operations are consistent with medical and professional standards of care.
- Recommends potential growth opportunities for new or existing services.
- Meets with the Medical Director or designated to discuss quality of patient care, review policies, procedures, outcomes reports, and records.
- Participates in quality management processes including encouragement of clinical support staff to assure proficiency in the delivery of clinical support activities.
- Calls patients to ensure their well-being as needed following center visits.
- In the absence of Medical Director, provides guidance to other medical personnel and center colleagues
Minimum Qualifications:
- Active, unrestricted MD or DO license in Pennsylvania.
- Experience in primary healthcare is preferred; geriatric certification is a plus.
- Strong organizational skills and ability to collaborate with clinical team members, as well as independently.
- Familiarity with telehealth platforms, billing systems, coding standards, and care coordination programs is desired.
- Commitment to compliance with licensing, certification, and accrediting bodies.
