Nurse Practitioner – Primary Care (with Psychiatric Experience)

Our client is a growing primary care company that meets the needs of Assisted Living and Skilled Nursing Facility clients. Onsite care by our energetic, state-of-the-art care providers addresses the unique and often complicated health needs of our patients.

We are looking for a Family Nurse Practitioner with a background in primary care and psychiatric/behavioral health to join our team in the Reading, PA area. This position offers flexibility in scheduling and the opportunity for incentive payments.

REQUIREMENTS

• Graduate of a Certified NP program

• Active and unrestricted NP license required, in Pennsylvania

• Minimum of at least 1+ years of primary care experience, working independently in an internal medicine or family practice setting

• Certified Nurse Practitioner (Required)

• Certifications: FNP-C

• Primary care (Required), Internal Medicine

• Background or experience in psychiatric/behavioral health strongly preferred

Experience

• Dedicated and passionate about helping people, especially the elderly

• Excellent communication skills to collaborate with patients, facility management, and internal stakeholders

• Strong problem-solving skills and ability to present solutions

• Skilled at addressing complex care needs, including co-occurring behavioral health conditions

• Exceptional documentation skills—detailed and accurate use of EMR systems

• Entrepreneurial mindset—this is not a directly supervised position within a practice, so you must be a self-starter who can work independently

• Motivated to find the best care options for our clients
