Job Description

A growing Home Health & Hospice organization in the greater Concord area is looking to hire a Director of Clinical Operations. This is an exciting opportunity to step into a high impact leadership role with a mission driven team committed to delivering exceptional patient care.

This role will be responsible for building, guiding, and elevating clinical programs ensuring every patient receives compassionate, high quality and compliant care. The Clinical Director will provide strategic and operational leadership for all Home Health and Hospice clinical services, overseeing clinical staff, patient care delivery, regulatory compliance, and quality outcomes across the organization. As a key member of the leadership team, this person will help shape the future of the agency.

Responsibilities:

Clinical leadership & oversight

  • Lead and oversee all Home Health and Hospice clinical staff and services
  • Direct patient care operations, including admissions, case assignments, and care coordination
  • Ensure patient needs are continually assessed and individualized plans of care are developed, implemented, and updated
  • Provide mentorship, coaching, and performance management to clinical team members
  • Facilitate interdisciplinary team collaboration and case conferences

Clinical operations & compliance

  • Ensure delivery of safe, high-quality care in compliance with all federal, state, and local regulations
  • Develop, implement, and oversee clinical policies and procedures
  • Monitor documentation, clinical outcomes, and key performance indicators
  • Work with QA & HR to design, implement, and evaluate orientation, training, and evaluation of clinical staff

Strategic leadership & growth

  • Partner with executive leadership to develop and achieve organizational goals
  • Support program development and service line growth for Home Health and Hospice
  • Build and maintain strong relationships within the healthcare community to promote referrals
  • Foster a culture of accountability, collaboration, and mission-driven care

Qualifications:

  • Active Registered Nurse (RN) license in New Hampshire, in good standing
  • Minimum 3–5 years of Home Health and/or Hospice experience — required
  • Minimum 3 years in a clinical leadership role (Director, Manager, or Supervisor)
  • Strong knowledge of Home Health & Hospice regulations, including CHAP standards
  • Experience with OASIS and hospice documentation requirements
  • Demonstrated ability to lead teams, manage operations, and drive outcomes
  • Excellent communication, leadership, and organizational skills
  • Reliable transportation, valid driver's license, and current CPR certification
