A dedicated nonprofit hospital committed to improving the health and well-being of its community is searching for a Nurse Practitioner for its Peterborough, NH location. This desirable healthcare facility is situated in the scenic region of southwestern New Hampshire, which boasts outdoor activities, local boutique shopping, a thriving arts scene, and many community engagement opportunities.

In this role, you will play a pivotal role in providing primary care services to patients across multiple sites, collaborating closely with the Medical Director and Director of Medical Practice Operations. Responsibilities:

• Conduct thorough assessments and collaborate on personalized, patient-centered care plans.
• Diagnose and treat illnesses, prescribing medications as needed.
• Deliver preventive care services such as screenings and patient education.
• Maintain accurate and compliant electronic health records.
• Collaborate with healthcare professionals to achieve optimal patient outcomes.
• Participate in quality improvement initiatives to enhance patient care practices.

Qualifications:

• Master's degree or higher required.
• Active Nurse Practitioner license required.
• 1+ years of experience in a clinical or practice setting; geriatric care experience preferred.

Additional Info:

• Full-time, permanent position
• $115,000–$130,000 depending on experience
