Job Description

A respected healthcare organization in Central New York is seeking a Licensed Practical Nurse (LPN) to join its Primary Care team. Opportunities are available in both Oswego and Fulton, NY. This full-time, day-shift position offers the opportunity to provide patient-centered care in an outpatient setting while working alongside physicians, advanced practice providers, nurses, and other members of the healthcare team.

Responsibilities:

• Assist in providing and evaluating patient care as a member of the primary care team.
• Support physicians and other providers with the delivery of high-quality patient care.
• Administer medications in accordance with established policies, procedures, and standards of care.
• Prepare and administer medications through appropriate routes and maintain accurate documentation.
• Observe patients for reactions to medications, treatments, or changes in condition and report concerns appropriately.
• Assist with patient mobility and maintain a safe, comfortable clinical environment.
• Follow infection prevention, aseptic technique, and equipment sterilization guidelines.
• Maintain appropriate levels of clinical supplies and routinely check emergency equipment.
• Accurately document patient care and utilize electronic systems as required.
• Communicate effectively and compassionately with patients, families, providers, and other members of the healthcare team.

Qualifications:

• Current New York State LPN license.
• Current CPR certification.
• One year of experience in a medical office or outpatient setting preferred.
• Proficiency in medication administration and phlebotomy preferred.
• Strong communication, organization, and patient-service skills.
• Ability to work effectively within a collaborative healthcare team.
