Job Description

A reputable home health and hospice organization in Southern New Hampshire is seeking a Home Health Registered Nurse for its Merrimack territory. In this role, you will provide compassionate care in a home setting for patients and families with needs ranging from basic to complex, utilizing the nursing process.

Key Responsibilities:

• Work one-on-one with patients outside of a clinical setting.
• Complete initial assessments of patients to determine individual needs.
• Develop and implement an appropriate plan of care, making adjustments as needed.
• Administer medication as directed.
• Provide ongoing education and support to patients and families.
• Document care provided and the patient’s progress toward desired outcomes.

Qualifications:

• Graduate of an accredited school of nursing.
• Active RN license.
• 1 year of acute care experience is required.

Position Details:

• Full-time, permanent
• Day Shift: 8 AM-5 PM
