A highly reputable home healthcare organization is searching for a Registered Nurse Case Manager for its Springfield Massachusetts territory. In this role, you will develop and implement individualized care plans and deliver quality nursing care in the home setting. You will effectively collaborate with patients, caregivers, and other healthcare team members while maintaining the standards of professional nursing. This is an exciting opportunity for you to change patients' and families’ lives and work in an autonomous environment.

Responsibilities:

• Work one-on-one with patients and families outside of a clinical setting.
• Develop and implement an appropriate plan of care that aligns with the physician’s orders.
• Provide ongoing education and support to patients and families.
• Utilizing technology to scale the impact of your time, communicate with the care team, and interact with families.

Qualifications:

• Graduate from an accredited nursing program.
• Active RN License.
• 2+ years of RN experience is required.
• 1+ years of home health or case management experience is preferred.

Additional info:

• Full-time, permanent position
• Monday-Friday, 8a-5p
• $65,000-82,000, depending on experience
