Job Description

A respected healthcare organization in Central New York is seeking a Licensed Practical Nurse (LPN) to join its team at a skilled nursing and rehabilitation facility. This full-time opportunity is available on day and evening shifts and offers competitive compensation, a $7,000 sign-on bonus, and the opportunity to provide hands-on, resident-centered care as part of an experienced nursing team.

Responsibilities:

• Provide direct nursing care to an assigned group of residents in accordance with individualized care plans.
• Administer medications, treatments, and procedures according to facility policies and safe nursing practices.
• Observe and evaluate residents for changes in condition and communicate concerns to the supervising RN or physician as appropriate.
• Direct and support CNAs in the delivery of resident care and completion of assigned duties.
• Complete accurate and timely documentation of resident care, treatments, changes in condition, and other required information.
• Coordinate unit operations when serving in a charge capacity, including resident assignments, staff breaks, rounds, and oversight of assigned duties.
• Assist with admissions, discharges, physician and Nurse Practitioner rounds, and resident/family education as directed.
• Provide preventative skin care and support residents' physical, emotional, and environmental safety.
• Maintain resident confidentiality and follow all applicable nursing policies, procedures, and regulatory requirements.

Qualifications:

• Graduate of an approved Licensed Practical Nurse (LPN) program.
• Current New York State LPN license required.
• Current CPR certification preferred.
• 6 months to 1 year of experience in a similar healthcare setting preferred.
• Proficiency in medication administration and phlebotomy skills.
• Strong communication, time management, problem-solving, and conflict-resolution skills.
• Knowledge of resident rights, abuse regulations, and the healthcare needs of an aging population.

Additional Info:

• Full-time, permanent position.
• Day shift: 6:00 AM–2:00 PM, including every other weekend.
• Evening shift: 2:00 PM–10:00 PM, including every other weekend.
• Pay: $25.25–$30.30/hour.
• Sign-on bonus: $7,000.
