A renowned Magnet healthcare system in New York is searching for a Registered Nurse Case Manager for one of its hospitals in White Plains. In this role, you will perform the initial assessment of admissions and screen patients for high-risk indicators to ensure patients receive the appropriate services for discharge to prevent readmission.

Responsibilities:

• Perform all activities for multidisciplinary care coordination at the intra- and inter-hospital levels of care.
• Perform all transitional planning activities, including patient/caregiving education, arrangement of aftercare, commercial payer involvement, and high utilizer patient root cause.
• Communicate and collaborate all relevant patient information to appropriate health team members.
• Ensure department goals of LOS reduction, readmission prevention, denial reduction, and satisfaction improvement are realized.
• Perform all UR activities, including commercial payer reviews, disposition determination, retro/self-audit, MC variance, and admission clinical review.

Qualifications:

• A BSN is required.
• Active RN license.
• 5+ years of ED, critical care, or med/surg experience is required.
• Prior case management experience is preferred.

Additional info:

• Full-time, permanent position
• Hours; Monday-Friday, 9a-5p + occasional weekends.
• Compensation: $100,000-140,000, depending on experience.
