A highly reputable home health and hospice organization in New Hampshire is searching for a Registered Nurse Case Manager for its Littleton/Whitefield territory. In this role, you will provide direct patient care, complete initial and ongoing assessments, plan & document care, and communicate with other healthcare providers and community resources as needed. This is an exciting opportunity for you to change patients’ lives and work in an autonomous environment.

Responsibilities:

• Deliver quality care to high-acuity patients, including IV infusions, wound vacs, and pain control.
• Work one-on-one with patients outside of a clinical setting.
• Design, implement, monitor, and evaluate patients’ care plans.
• Coordinate services and inform MDs and necessary healthcare professionals of changes in patient’s conditions.
• Provide ongoing education and support to patients and families.
• Administer medications as needed or directed.

Qualifications:

• Graduate from an accredited nursing program.
• A BSN is preferred.
• Active RN License
• 2+ years of home health experience is preferred.

Additional info:

• Full-time, permanent position
• Schedule: Monday-Friday, days
• Compensation: $33-53/hr. base, depending on experience, + incentives!
