A highly reputable home health and hospice organization in New Hampshire is seeking an experienced and compassionate Registered Nurse Case Manager to serve patients in the Littleton, NH territory. As an RN Case Manager, you’ll provide direct, holistic care and serve as a trusted partner to patients, families, and the healthcare team.

Key Responsibilities

• Deliver high-quality care to patients with complex needs, including IV infusions, wound vacs, and pain management.
• Conduct comprehensive initial and ongoing assessments to develop individualized care plans.
• Coordinate care and communicate effectively with physicians, healthcare professionals, and community resources.
• Provide education, guidance, and emotional support to patients and their families.
• Administer medications and treatments as prescribed.
• Promote patient independence and support positive health outcomes through compassionate, evidence-based care.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Active RN license.
• 2+ years of home health experience preferred.
• Must be comfortable working independently and traveling within the assigned territory.

Position Details

• Full-Time, Permanent
• Schedule: Monday-Friday, Day Shift
• Territory Travel: Approximately 50-60 miles per day
