Drive the change in healthcare! A Magnet-recognized healthcare system in Massachusetts is searching for Home Health Registered Nurses from its Medford/Malden and Winchester/Stoneham territories. In this role, you will provide compassionate care in a home setting to patients and families with needs ranging from basic to complex utilizing the nursing process.

Responsibilities:

• Work one-on-one with patients outside of a clinical setting.
• Complete initial assessments of patients to determine individual needs.
• Develop and implement an appropriate plan of care and make changes as needed.
• Administer medication as directed.
• Provide ongoing education and support to patients and families.
• Document care provided and the patient’s progress toward desired outcomes.

Qualifications:

• An associate’s degree is required, and a BSN is preferred.
• Active RN license.
• 1+ years of nursing experience is required.
• Prior home health experience is preferred.

Additional Info:

• Full-time, permanent position
• Schedule: Monday-Friday, 8a-430p
• $43-52/hr. base, depending on experience, + incentives!
