A Magnet-recognized healthcare system in Massachusetts is seeking a compassionate and skilled Registered Nurse to join its growing Home Care team. In this role, you will care for patients with a wide range of needs, from basic to complex, using the nursing process to promote optimal outcomes. The position involves regular travel within a small, manageable territory, including Watertown and surrounding communities.

Key Responsibilities

• Provide individualized, one-on-one nursing care in patients’ homes
• Conduct comprehensive initial and ongoing patient assessments
• Develop, implement, and modify personalized plans of care
• Administer medications and treatments as prescribed
• Educate and support patients and their families to promote independence and safety
• Accurately document care provided and patient progress toward goals

Qualifications

• Associate Degree in Nursing required; BSN preferred
• Active RN license
• Minimum of 1 year of nursing experience
• Home health experience preferred, but not required
