Case Management Director – Charlotte, NC

We are seeking a passionate and experienced individual to join our team as the Director of Case Management at our outstanding health system Medical Center. As a part of our organization, you will have the opportunity to make a meaningful impact on the lives of our patients and their families while working in a supportive and caring environment.

• As the Director of Case Management, you will lead and oversee care coordination activities, utilization review, social services, and discharge planning at our hospital. You will work closely with the medical staff and hospital leadership to develop care management programs that efficiently use medical resources and provide quality patient care.

• To be successful in this Director of Case Management role, you must possess a Bachelor's degree in Nursing, Health Care Administration, or a related field, with a BSN or LMSW required. Accredited Case Manager (ACM) certification and an MSN are preferred.
