Family Practice Nurse Practitioner – Private Family Medicine Practice – Great location near Orland Park in Southwest Chicago!

Our well-established and respected solo private Family Medicine practice is currently seeking an experienced Family Practice Nurse Practitioner to join our team. As a Family Practice Nurse Practitioner, you will join our team and play a crucial role in providing comprehensive medical care to patients of all ages, promoting wellness, and managing chronic conditions within a family practice setting.

Responsibilities:

  • Conduct thorough patient assessments, including medical history reviews, physical examinations, and diagnostic testing when necessary
  • Develop and implement customized treatment plans, including prescribing medications, administering treatments, and providing patient education
  • Manage and coordinate care for patients with chronic conditions, such as diabetes, hypertension, and asthma
  • Collaborate with physicians and other healthcare professionals to ensure holistic and patient-centered care
  • Provide counseling and support to patients and their families, emphasizing preventive care and healthy lifestyle choices
  • Maintain accurate and up-to-date electronic medical records (EMRs) and documentation of patient encounters
  • Stay current with the latest medical advancements, treatment guidelines, and best practices in family practice nursing

Requirements:

  • Valid Nurse Practitioner license in the State of Illinois
  • Master's degree in Nursing with a focus on Family Practice preferred
  • Minimum of 3 years of experience as a Family Practice Nurse Practitioner preferred
  • Excellent clinical assessment, diagnostic, and treatment skills
  • Strong knowledge of family medicine principles, primary care practices, and evidence-based medicine preferred
  • Proficiency in using electronic medical record (EMR) systems (eClinicalWorks)
  • Outstanding interpersonal and communication skills, with the ability to effectively communicate complex medical information to patients and their families
  • Compassionate, patient-centered approach with a commitment to delivering high-quality care
