POSITION OVERVIEW
We are seeking an experienced and highly skilled Nurse Director of Case Management to lead and oversee our organization's case management department. As the Nurse Director, you will be responsible for developing and implementing strategies, policies, and procedures to ensure efficient and effective case management services are provided to our patients. Your strong leadership abilities, clinical expertise, and deep understanding of case management principles will be crucial in driving optimal patient outcomes and achieving organizational goals.
RESPONSIBILITIES
Leadership and Team Management:
Provide strategic direction and leadership to the case management department, ensuring the delivery of high-quality, patient-centered care.
Supervise and mentor a team of case managers, ensuring appropriate caseload distribution, adherence to best practices, and professional development opportunities.
Foster a collaborative and supportive work environment that encourages teamwork, open communication, and continuous improvement.
Program Development and Implementation:
Develop and implement case management policies, procedures, and protocols in compliance with regulatory requirements and industry standards.
Identify opportunities for program enhancement, expansion, and optimization to enhance patient care coordination, improve outcomes, and control healthcare costs.
Collaborate with interdisciplinary teams to integrate case management services into patient care plans and ensure smooth transitions across healthcare settings.
Quality Improvement:
Establish and monitor quality metrics and performance indicators to assess the effectiveness and efficiency of case management interventions.
Analyze data and trends to identify areas for improvement and develop strategies to enhance patient outcomes, reduce readmissions, and minimize variations in care.
Lead quality improvement initiatives, promoting evidence-based practices and best-in-class care delivery models.
Patient Advocacy and Education:
Advocate for patients and their families, ensuring their needs, preferences, and rights are respected throughout the care continuum.
Provide education and resources to patients, families, and staff on case management principles, care transitions, and available community resources.
Collaborate with community organizations and healthcare providers to promote seamless care coordination and access to appropriate services.
Collaboration and Communication:
Foster strong relationships and effective communication channels with key stakeholders, including physicians, nursing staff, ancillary departments, and external partners.
Collaborate with finance and revenue cycle teams to ensure accurate and timely documentation, coding, and billing of case management services.
Represent the organization in professional associations, committees, and conferences to stay abreast of industry trends and contribute to the advancement of case management practices.
QUALIFICATIONS
EDUCATION
Bachelor's degree required
Master's Degree required or in process and must obtain within 2 years of hire
LICENSURE/CERTIFICATION/REGISTRATION
Licensed Registered Nurse in the state of Florida or Multi-State License
